The End of Paper Records
Historically, patient health records were created and stored on paper with each individual health care provider. Patient intake forms, chart notes, including diagnosis and treatment plan, along with lab results, would all be gathered in a patient file and housed in large filing cabinets.
Access to these types of files required requesting permission, usually in writing and then copies would be made and provided to the patient. More recently, providers had access to scanning capabilities and were able to scan patient records and store them electronically in a patient database.
In order for a patient to access these records, a request must be made and copies were printed and provided to the patient. Keeping up to date on all records requires continually collecting your updated records from various providers and organizing them either on paper or electronically.
Very few people actually maintain and track their health records in this way. They leave their health records in the hands of the providers, scattered over time and location, with no tracking of their health progress over time or use of the data in a proactive way.
